Practice Transformation
The KY AETC’s PT program’s goals are to increase the size and strength of the HIV clinical workforce and to improve patient outcomes along the HIV Care Continuum. We partner with selected clinics across Kentucky to improve patient outcomes by integrating principles of the patient-centered medical home model and implementing HIV care and behavioral health services.
5-Year Partnerships with Community Health
KY AETC partners with community health care clinics for five years with the goal of transforming selected clinical practices and building the capacity of the selected organizations to provide quality HIV care. Our PT program coordinator works closely with each clinic to identify its needs and tailor trainings that work for its providers.
Trainings for Health Care Providers
Additionally, the Kentucky Program partners with KY AETC and the Kentucky AIDS Drug Assistance Program (KADAP) Income Reinvestment Program to offer training to health care providers across the state that would like to learn more about HIV and the care continuum, in order to better serve their communities.
