Kentucky Homeplace Referral

Kentucky Homeplace has community health workers (CHWs) in 32 counties across eastern Kentucky. CHWs complete special training and certification to provide clients with important health information to help individuals pursue a healthy lifestyle.  We also provide support to help clients navigate and access a wide variety of health and social services.
