Residency and Fellowship Verification Requests
About
GME can verify residency and fellowship training programs. All verification requests must be in writing and include a signed release.
You may send requests to UK GME Verification Portal.
Use the portal to request training verification for trainees from 1998 to the present. For verification requests for a trainee who completed training prior to 1998, email your request to gmerecords@uky.edu.
GME can only verify dates of training. If you have questions about performance/privileges during training, please contact the individual program.
The GME Office does not complete the following forms:
- Medical Board Licensure Forms
- Detailed Institutional Questionnaires
- FCVS requests

Pharmacy Requests

To verify a Doctor of Medicine (MD) degree, send request to University of Kentucky College of Medicine Registrar at med.registrar@uky.edu.
Verify a DMD Degree
To verify a Doctor of Dental Medicine (DMD) degree, send request to University of Kentucky College of Dentistry Registrar at missy.shelton@uky.edu.
