Licensure

• Required for current PGY-2+ residents or residents who have had 12+ months of post-graduate accredited training.
• If you are an incoming PGY-2+ with more than 24+ months of training, please follow the application steps below for "Regular License (Full)".
• Incoming/current UK PGY-1 residents: The necessary licensure forms are completed during your onboarding process or at orientation. The GME office will facilitate the application process and mail out your license paperwork in the spring of your PGY-1 year.  Please watch for these instructions via e-mail.   

• A resident with an R license is eligible to moonlight or obtain a DEA. 

• Issued to a resident who has completed 12+ months of post graduate accredited training and has passed parts 1 and 2 of USMLE or COMLEX but has not passed part 3.
• Issued to a resident who has completed 12+ months of post graduate accredited training and has passed all parts of USMLE or COMLEX, but their scores were not processed in time for the KBML board meeting when the license was reviewed.

• IP licenses limits medical practice to the parameters of a training program.  A resident with an IP license is not eligible to Moonlight or obtain a DEA.

• Required for fellows and non-accredited chiefs. 
• Required for incoming residents who have had 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX.
• Physicians with 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX, are eligible to apply.

• A physician who holds a Regular (Full) License is eligible to moonlight and obtain a DEA. 

• Issued to an applicant who meets the requirements for a regular (full) physician license and needs to start working in Kentucky before the next board meeting.
• An application for a regular (full) license must be completed and submitted to the board before a TP license is issued. 
• Permit is only valid for six months.

• Issued to a physician who has been accepted into a Kentucky Fellowship Training Program and do not meet the requirements for a Regular (Full) License OR has graduated from a medical school located outside of the United States or Canada. 
• FT Licenses limits medical practice to the parameters of a training program. A physician with an FT License is not eligible to Moonlight or obtain a DEA.

• Before applying for an FT license, please reach out to the GME office for guidance on which license type you will need for your fellowship. 

To obtain your initial Kentucky Training License, you must complete the application processes through both KBML and FCVS outlined below. 

This License pathway applies to:

• PGY-2 residents, or
• Residents who have completed between 12-24 months of accredited postgraduate training and have passing USMLE/COMLEX parts 1-2 or USMLE/COMLEX parts 1-3.

IMPORTANT:

• Incoming PGY-2+ residents who will have 24 or more months of training completed by your start date and passing USMLE/COMLEX parts 1-3, should instead follow the application steps for a Full (Regular) Physician License. 
• Incoming/current UK PGY-1 residents: Please do not apply independently. Licensure forms are completed during your onboarding process or orientation. The GME office facilitates the application process and mails your KBML paperwork during the spring of your PGY-1 year. Please watch for these instructions via email.

KBML Application Instructions:

1. Create an online account with KBML: eBusiness: Login (ky.gov) 

   • Select "Create an Account". 
   • Username Format: Last name + first initial (Example: SmithJ)
   • Password: Last four digits of your SSN

2. Complete the Online Application- Select 'Apply for a License'

3. Pay the Application Fee- Submit the $75 application fee online after completing your application.

4. Complete the Printable Application Forms from your KBML Account (will need to be notarized and mailed)

   • Program Director Approval Form- the GME office will work with your program to complete and submit on your behalf

5. Mail the paper forms to KBML:

   • Mail to: Kentucky Board Of Medical Licensure - 310 Whittington Parkway, Suite 1B, Louisville, KY 40222    

     6. Submit an FCVS Credentialing Application (See detailed Instructions below).

FCVS Credentialing Application Instructions:

IMPORTANT: 

• FCVS processing typically takes 6-8 weeks. Delays in completing this step may impact your ability to obtain your Kentucky training license.
• If you are applying for your initial license for your upcoming PGY-2 training year- Do not submit your FCVS application before April 1 or before 90 days of completing your current PGY-1 year. KBML cannot accept your FCVS profile before 90 days of completing 12 months of training.
• You must submit the FCVS application regardless of whether you have received USMLE Step 3 or COMLEX Level 3 scores. If your exam scores are received after submitting your FCVS application, or you have not taken step/level 3, please contact the GME office to determine your next steps.
• KBML will not accept credentials or exam scores from any source other than FCVS. Please designate the Kentucky Board of Medical Licensure as the Recipient of your FCVS profile.

• Estimated FCVS Application Fee: $400-600

• Submit an FCVS credentialing application at:

  •  https://www.fsmb.org/fcvs/ 

    • Select Kentucky Board of Medical Licensure as your recipient during submission.

To switch your IP license to an R license, you will need to follow and complete the instructions below.

• Residents are eligible to switch their license to a Residency (R) License if you have a passing Step/Level 3 score.
• If applying to switch to an R license within 6 months of your IP license being issued, KBML will issue your R license outside of board meetings.
• If applying to switch to an R license after 6 months of your IP license being issued, KBML will review and issue your R license at one of the quarterly board meetings.

KBML Instructions:

**Do NOT apply online**

1. Print and fill out 'Residency Form- Changing IP to R license' forms. 

   1. Forms can be found here: Apply For A License - Kentucky Board of Medical Licensure

   2. Please note that page 1 of the 'Residency Form- Changing IP to R license' form will need to be signed by your program director.

   3. Form will need to be notarized

2. Pay application Fee: $75 

   1. Mail Credit Card/Check form: Please e-mail your KBML Licensure Coordinator or GMEAdmin@uky.edu for a copy of the payment form.

3. Mail forms to Kentucky Board of Medical Licensure:

4. Send your Step or Level 3 score to KBML (see detailed instructions below)

Instructions to send your USMLE Step 3/COMLEX Level 3:

Processing may take 6-8 weeks

• USMLE - An updated transcript will need to be sent directly from FCVS to the KY Medical Board here https://www.fsmb.org/transcripts/ 
• COMLEX - You will need to reach out to NBOME directly to have your Level 3 transcript sent to the KY Medical Board.

To obtain your Kentucky Full (Regular) Physician License, you must complete all parts of the application process outlined below.

This License pathway applies to:

• Fellows and Non-accredited Chiefs who have completed 24 or more months of accredited postgraduate training and have passed all parts of USMLE or COMLEX
• Incoming PGY-2+ Residents who will have 24 or more months of accredited postgraduate training by their start date and have passed all parts of USMLE or COMLEX

IMPORTANT:

• If you do not meet the KY Full (Regular) Physician license requirements, please contact the GME office to confirm which license is appropriate for your training.

KBML Application Instructions:

1. Login or Create a KBML Account:
   • For physicians who hold a current KY license:
     • Login to your KBML profile: eBusiness: Login (ky.gov) (Username: current license number, Password: Last 4 of SSN)
   • For incoming physicians who do not hold a current KY license:
     • Create an account with KBML: eBusiness: Login (ky.gov)
     • Select "Create an Account". 
       • Username Format: Last name + first initial (Example: SmithJ)
       • Password: Last four digits of your SSN
2.  Complete the Online Application- Select 'Apply for a License'
   • IMPORTANT: Select the type of license/certificate you will be applying for in Kentucky:  'Physician'
3. Pay KBML Licensing fee online or mail in credit card/check form 
   • Physicians who completed medical school and residency in KY: $175 fee
     • Choose the option online to bill fee and complete the credit card/check form to mail in with your application forms.
     • Please e-mail your KBML Licensure Coordinator or GMEAdmin@uky.edu for a copy of the credit card/check form.
   • Physicians who completed medical school and/or residency outside of KY: $300 fee
     • You can pay this fee online 
4. Complete the Printable Application Forms from your KBML Account (will need to be notarized and mailed)
   • Please note the Temporary Permit Form is not necessary unless you need a license issued before a KBML board meeting date.
5. Mail your completed and notarized paper forms to KBML: 
   • Kentucky Board of Medical Licensure, 310 Whittington Parkway, Suite 1B, Louisville, KY 40222    
6. Complete a criminal background check. Instructions can be located in the paper forms.
   • IMPORTANT: if you are applying for a Full license for your upcoming PGY-3 training year, please do not complete this step earlier than 90 days before completion of your current PGY-2 year.  
7. Submit an FCVS Credentialing Application (detailed FCVS instructions below)

Before applying, please work with the GME office and your KBML Licensure Coordinator to confirm the FT license is the appropriate license for your training.

The FT License is a restricted license and only issued to Fellows who do not meet the following requirements for the KY Regular (Full) Physician License:

• The Fellow has not completed 24 or more months of U.S. accredited training 
• The Fellow has not passed all parts of USMLE/COMLEX
