Residency Training License (R)

  • Required for current PGY-2+ residents or residents who have had 12+ months of post graduate accredited training.
  • If you are an incoming PGY-2+ with more than 24+ months of training, please follow the application steps below for "Regular License (Full)".
  • Incoming/current UK PGY-1 residents: The necessary licensure forms are completed during your onboarding process or at orientation. The GME office will facilitate the application process and mail out your license paperwork in the spring of your PGY-1 year.  Please watch for these instructions via e-mail.   
  • A resident with an R license is eligible to Moonlight or obtain a DEA. 

Institutional Practice License (IP)

  • Issued to a resident who has completed 12+ months of post graduate accredited training and has passed parts 1 and 2 of USMLE or COMLEX but has not passed part 3.
  • Issued to a resident who has completed 12+ months of post graduate accredited training and has passed all parts of USMLE or COMLEX, but their scores were not processed in time for the KBML board meeting when the license was reviewed.
  • IP licenses limits medical practice to the parameters of a training program.  A resident with an IP license is not eligible to Moonlight or obtain a DEA.

Regular (Full) Physician License

  • Required for fellows and non-accredited chiefs. 
  • Required for incoming residents who have had 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX.
  • Physicians with 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX, are eligible to apply.
  • A physician who holds a Regular (Full) License is eligible to moonlight and obtain a DEA. 

Temporary Permit (TP)

  • Issued to an applicant who meets the requirements for a regular (full) physician license and needs to start working in Kentucky before the next board meeting.
  • An application for a regular (full) license must be completed and submitted to the board before a TP license is issued. 
  • Permit is only valid for six months.

Fellowship Training License (FT)

  • Issued to a physician who has been accepted into a Kentucky Fellowship Training Program and do not meet the requirements for a Regular (Full) License OR has graduated from a medical school located outside of the United States or Canada. 
  • FT Licenses limits medical practice to the parameters of a training program. A physician with an FT License is not eligible to Moonlight or obtain a DEA.
  • Before applying for an FT license, please reach out to the GME office for guidance on which license type you will need for your fellowship.

Applying for Initial KY R or IP Training License

To obtain your initial Kentucky training license, you will need to follow and complete the KBML and FCVS instructions below. 

  • Required for current and incoming PGY-2+ residents or residents who have had 12+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX.
  • If you are an incoming PGY-2+ with more than 24 months of training, please follow the application steps below for "Regular License (Full)".
  • Incoming/current UK PGY-1 residents: Please contact the GME office before applying for a license. The necessary licensure forms are completed during your onboarding process or at orientation. The GME office will facilitate the application process and mail out your KBML/FCVS license paperwork in the spring of your PGY-1 year.  Please watch for these instructions via e-mail.   
 
KBML
IMPORTANT: 
  • There are two parts of the KBML application process- apply online AND mail a paper application, affidavit, and PD approval form (links to forms are listed below). Please start the KBML application process as soon as possible, as this can take several weeks to process. 
 
Application instructions:
  1. Create an account with KBML: eBusiness: Login (ky.gov) 
    • click "Create an Account". 
    • Username: Enter your last name followed by your first initial (i.e., smithj for John Smith)
    • Password: Enter the last four digits of your social security number
  2. Once your profile is created, choose "Apply for a license" and fill out the online application.
  3. Pay for the application fee.
  4. Fill out the printable application and affidavit forms.  Copy of forms here: KBML R License Affidavit and Application Appendix_1.pdf (uky.edu). 
  5. Fill out your information on the Program Director Approval Form and then the form will need to be signed by your program director.                             Here is a copy of the form: R License PD Approval Form.pdf
    • You can mail this form separate from your application and affidavit. 
    • Please reach out to GME (GMEadmin@uky.edu) or your program coordinator if you need help with getting the PD form signed. 
  6. Mail the application, affidavit, and PD form to KBML:

FCVS
IMPORTANT: 
  • This may take six to eight weeks to process and get to KBML Failure to successfully complete this application in a timely manner will impact your ability to obtain a Kentucky medical license.
  • If you are applying for your initial license for the upcoming PGY-2 training year, do not complete this step before April 1st. However, it should be promptly completed at the beginning of April. KBML will not accept your FCVS transcripts before 90 days prior to your PGY-2 training year. 
  • The FCVS application needs to be submitted regardless of having received your Step 3 and/or COMLEX 3 scores. If you receive your scores after submitting your application or have not yet taken the exam, please let the GME office know ASAP regarding next steps. If your Step 3/COMLEX 3 scores are not received prior to the KBML June Board meeting, you will be issued an IP (limited) license instead of an R (residency training license).
 
Application Instructions:
  1. Complete an FCVS credentialing application with FSMB (Federation of State Medical Boards): My FSMB - Home
  2. Select 'Medical Board- Kentucky' as the recipient of your application. Failure to send this directly to KBML will result in needing to submit/pay for a secondary application. 
  3. Pay fee. List of cost and fees can be found here: FSMB | Cost and Fees
  4. Print and fill out the FCVS Affidavit and Release forms, these will need to be notarized. Forms and instructions can be found here: mail-in-option.pdf (fsmb.org)
    • You do have the option to complete these forms online with an online notary.  Instructions can be found here: FSMB | Affidavit/CID Forms - Notary Options
    • If you are a currently UK PGY-1, this paperwork was completed at orientation. Reach out to the GME Office if you need a copy.
  5. Mail to FSMB or upload to your online FCVS application: Notarized Affidavit, Release and copy of birth certificate or passport. 
    • If your last name is different than what is listed on your birth certificate or passport, you will need to mail in a copy of your marriage certificate.

Applying for an R license if you currently hold an IP license

To obtain your Kentucky Residency training license, you will need to follow and complete the instructions below.

  • PGY-2+ residents are eligible to apply after passing part 3 USMLE or COMLEX.
  • If applying for your Residency license within 6 months of your IP license being issued, KBML will issue your R license outside of board meeting dates.
  • If applying for your Residency license after 6 months of your IP license being issued, you will have to wait for a KBML board meeting for your R license application to be reviewed and an R license issued. 
 
KBML
IMPORTANT: 
  • DO NOT APPLY ONLINE when upgrading from IP to R license
 
Application Instructions:
  1. Print and fill out  'Residency Form- Changing IP to R license' forms. 
    1. Forms can be found here: Changing IP to R Forms.pdf (ky.gov)
      • Please note that page 1 of the 'Residency Form- Changing IP to R license' form will need to be signed by your program director.
      • Form will need to be notarized
      • Mail form to KBML

USMLE Step 3/COMLEX Level 3
IMPORTANT: 
  • This may take six to eight weeks to process and get to KBML. 
  • USMLE - You need ot have your transcript sent directly from FCVS to the KY Medical Board here https://www.fsmb.org/transcripts/ 
  • COMLEX - You need to reach out to NBOME directly to have your Level 3 transcript sent to the KY Medical Board.
 
 

 

Applying for a Regular (Full) License

To obtain your Kentucky Residency training license, you will need to follow and complete the KBML and FCVS instructions below.

  • Required for fellows and non-accredited chiefs who have completed more than 24 months of post graduate accredited training in the United States. 
  • Required for incoming residents who have had 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX.
  • Physicians with 24+ months of post graduate accredited training and have passed all parts of USMLE or COMLEX, are eligible to apply.
 
KBML
IMPORTANT: 
  • There are three parts of the KBML application process- apply online AND mail a paper application, (links to forms are listed below) and complete a criminal background check. 
 
Application instructions:
  1. Apply online:
    • For physicians who hold a current KY license:
    • For incoming physicians who do not hold a current KY license:
      • Create an account with KBML: eBusiness: Login (ky.gov)
      • Click "Create an Account". 
        • Username: Enter your last name followed by your first initial (i.e., smithj for John Smith)
        • Password: Enter the last four digits of your social security number
  2.  Click on 'Apply for a License'
    • Select the type of license/certificate you will be applying for in Kentucky:  'Physician'
  3. Complete the online application
  4. Pay fee online or mail in credit card/check form 
    • Physicians who completed medical school in KY: $175 fee
      • Choose the option online to bill fee and mail in the credit card/check form to KBML for $175.

    • Physicians who completed medical school outside of KY: $300 fee

    • Please note the Temporary Permit Form is not necessary unless you need a license issued before a KBML board meeting date.
  6. Complete a criminal background check. Instructions can be found here: KBML Background Check Information.pdf (uky.edu)
    • IMPORTANT: if you are applying for a Full license for the upcoming training year and currently do not have 24 months of training, please do not complete this step before 90 days prior to the upcoming training year.  
    • Please note: GME does not reimburse for the background check fee. 
  7. Mail in your completed and notarized paper application.

FCVS

    • Please note: FCVS transcripts will need to be need sent, even if you have completed this step recently for another license application.
