Types of Licenses:
Residency Training License (R)
- Required for current PGY-2+ residents or residents who have had 12+ months of post-graduate accredited training.
- If you are an incoming PGY-2+ with more than 24+ months of training, please follow the application steps below for "Regular License (Full)".
- Incoming/current UK PGY-1 residents: The necessary licensure forms are completed during your onboarding process or at orientation. The GME office will facilitate the application process and mail out your license paperwork in the spring of your PGY-1 year. Please watch for these instructions via e-mail.
A resident with an R license is eligible to moonlight or obtain a DEA.
Residency Training License (R)
- Required for current PGY-2+ residents or residents who have had 12+ months of post-graduate accredited training.
- If you are an incoming PGY-2+ with more than 24+ months of training, please follow the application steps below for "Regular License (Full)".

IP licenses limits medical practice to the parameters of a training program. A resident with an IP license is not eligible to Moonlight or obtain a DEA.

Temporary Permit (TP)
- Issued to an applicant who meets the requirements for a regular (full) physician license and needs to start working in Kentucky before the next board meeting.
- An application for a regular (full) license must be completed and submitted to the board before a TP license is issued.
- Permit is only valid for six months.

Fellowship Training License (FT)

Applying for an Initial KY R or IP Training License
To obtain your initial Kentucky Training License, you must complete the application processes through both KBML and FCVS outlined below.
This License pathway applies to:
- PGY-2 residents, or
- Residents who have completed between 12-24 months of accredited postgraduate training and have passing USMLE/COMLEX parts 1-2 or USMLE/COMLEX parts 1-3.
IMPORTANT:
- Incoming PGY-2+ residents who will have 24 or more months of training completed by your start date and passing USMLE/COMLEX parts 1-3, should instead follow the application steps for a Full (Regular) Physician License.
- Incoming/current UK PGY-1 residents: Please do not apply independently. Licensure forms are completed during your onboarding process or orientation. The GME office facilitates the application process and mails your KBML paperwork during the spring of your PGY-1 year. Please watch for these instructions via email.
KBML Application Instructions:
Create an online account with KBML: eBusiness: Login (ky.gov)
- Select "Create an Account".
- Username Format: Last name + first initial (Example: SmithJ)
- Password: Last four digits of your SSN
Complete the Online Application- Select 'Apply for a License'
Pay the Application Fee- Submit the $75 application fee online after completing your application.
Complete the Printable Application Forms from your KBML Account (will need to be notarized and mailed)
- Program Director Approval Form- the GME office will work with your program to complete and submit on your behalf
Mail the paper forms to KBML:

IMPORTANT:
- FCVS processing typically takes 6-8 weeks. Delays in completing this step may impact your ability to obtain your Kentucky training license.
- If you are applying for your initial license for your upcoming PGY-2 training year- Do not submit your FCVS application before April 1 or before 90 days of completing your current PGY-1 year. KBML cannot accept your FCVS profile before 90 days of completing 12 months of training.
- You must submit the FCVS application regardless of whether you have received USMLE Step 3 or COMLEX Level 3 scores. If your exam scores are received after submitting your FCVS application, or you have not taken step/level 3, please contact the GME office to determine your next steps.
- KBML will not accept credentials or exam scores from any source other than FCVS. Please designate the Kentucky Board of Medical Licensure as the Recipient of your FCVS profile.

Switching your current KY Institutional Practice (IP) License to a Residency (R) License
To switch your IP license to an R license, you will need to follow and complete the instructions below.
- Residents are eligible to switch their license to a Residency (R) License if you have a passing Step/Level 3 score.
- If applying to switch to an R license within 6 months of your IP license being issued, KBML will issue your R license outside of board meetings.
- If applying to switch to an R license after 6 months of your IP license being issued, KBML will review and issue your R license at one of the quarterly board meetings.

- USMLE - An updated transcript will need to be sent directly from FCVS to the KY Medical Board here https://www.fsmb.org/transcripts/
- COMLEX - You will need to reach out to NBOME directly to have your Level 3 transcript sent to the KY Medical Board.
Switching your current KY Institutional Practice (IP) License to a Residency (R) License
