Report Concerns

We strive to create a community of support and accountability. The following reporting forms are important in helping us accomplish this. 

 

Bias Reporting 

UK is committed to ensure any and all instances of bias or discrimination are addressed, and the victims supported. If you or someone you know needs additional support, you can reach out to the Office for Institutional Diversity and complete a Bias Incident Report

  • Two different types of BIRT reports: INFORMATION ONLY or TAKE ACTION 

    • INFORMATION ONLY reports are used to track trends and help determine patterns across campus.  

    • TAKE ACTION reports are forwarded on to the appropriate University official for further review.
